Annual Group Information Form Updates

The online Annual Group Information Form (AGIF) tool (used for community rated groups only) has been updated as of September 29, 2018, in order to make the submission process quicker and simpler for Groups.

In addition, the paper form has been updated, as well as the instructions. 

Groups may begin using this version of the AGIF effective immediately, but we will accept the prior version for any signature dates until January 1, 2019.
