Annual Group Information Form (AGIF) Mailing Issue

On July 31, Univera Healthcare mailed an Annual Group Information Form (AGIF) request to small groups. Unfortunately, some small groups with January 2024 renewals received the wrong letter.

These groups should have received the initial notice to collect an AGIF, but instead they received the 90-day notice letter.

If one of your small groups has a January 2024 renewal and has not yet provided an AGIF, they will still need to provide an AGIF prior to January 1; however, their renewal is not in jeopardy at this time.
