Patients in Western New York who face gaps in medical care will soon have access to essential support and guidance, thanks to funding support that we provided to Evergreen Health. This initiative aims to assist individuals with complex health and behavioral health conditions, particularly those impacted by poverty, racial and ethnic disparities and other social determinants of health.
Through this program, patients will benefit from the expertise of an adherence manager who specializes in case management. This role focuses on direct patient care and will guide individuals in adhering to their health needs, including screenings, appointments and follow-up care.
Evergreen operates a comprehensive healthcare delivery system in Western New York, providing medical, supportive and behavioral health services through its community based Federally Qualified Health Centers.
